Breast augmentation

At Reformkliniken, we take pride in offering our patients the latest technology and methods in breast augmentation. With over 5000 successfully performed breast surgeries, we have the experience and expertise required to fulfill your wishes and goals.

For most people, a good result means not having to think about their breasts and that they feel like a completely natural part of the body.

Breasts are an important part of self-image but also crucial for how you want to present yourself to others. Feeling secure and comfortable in various situations, both with and without clothes, is easier when the breasts have a good shape and stability.

Depending on your conditions and desires, the breasts can become much larger, slightly larger, or not larger at all since it is possible to combine a reduction of the natural breast with the insertion of implants.

For most people, a good result means not having to think about the breasts and feeling that they are a completely natural part of the body.

The reasons women may feel dissatisfied can be due to congenital characteristics, such as having small, uneven, or perceived abnormal breasts, but it can also be the result of changes related to pregnancy, breastfeeding, or weight fluctuations for other reasons. The risk of complications is very low, and the material used in breast implants is more durable today than before.

This means that most people can achieve a good result with a procedure. However, the body changes over time and can be affected by pregnancy and weight changes even after surgery, so it is important to be aware that you may need additional procedures in the future.

If the breasts have a good shape and the skin is of good quality, inserting an implant may be sufficient. However, if the skin is stretched and the breasts are sagging, a breast lift may be required at the same time.

In cases where there is a significant difference in the size and shape of the breasts, known as “asymmetry,” it may sometimes be enough to use implants of different sizes, but a breast lift may also be necessary simultaneously.

Implants

Round vs. Teardrop-shaped

A round implant is like a circle at the base, the part placed against the chest wall, while the shape adapts to the available space. A teardrop-shaped implant has width, height, and projection and is designed to maintain a specific shape in the body.

For those seeking a natural result, a teardrop-shaped breast might be ideal, but a round implant can also achieve a teardrop shape in the body.

A round implant, for example, can be placed through a small incision in the armpit, whereas a teardrop-shaped implant requires a larger scar placed in the fold under the breast.

Implants

Silicone implants

Modern breast implants are made of a type of silicone known in Sweden as “geléhallon,” in Denmark as “skaerefast,” and in English-speaking countries as “gummy bear” or “cohesive.” This means the content is soft but not liquid. If the implant is damaged, the content stays in place instead of leaking out. This helps the breasts maintain their shape better and reduces the risk of complications.

When a foreign material is placed in the body, the healing process creates a membrane called a “pocket” that helps keep the implant in place and maintain the desired shape.

The pocket forms and stabilizes during the first four weeks but can be affected up to about three months after surgery. Therefore, the initial period after surgery is crucial for how the breasts will look in the future.

Typically, the pocket is thin and pliable, making the implants feel soft and natural. If the pocket becomes tight and thick, the breast will feel hard and uncomfortable. The pocket should be roomy enough for the implant to lie flat. If the pocket is too small and tight, the implant will be affected in shape and have folds on the surface. If the pocket becomes increasingly thick and tight, the shape will be more affected, and the breast will become harder until it feels like a small ball. This is called capsular contracture, a rare complication that can appear early after surgery or years later.

The surface of an implant can be smooth or textured. Textured implants can range from heavily roughened to almost entirely smooth to the touch.

A heavily textured surface is supposed to make the implant “grow in,” an interaction between the body and the implant, which theoretically keeps the implant better in place. Unfortunately, this does not work as well in practice, and the implants can develop a “double capsule,” allowing them to move freely inside the pocket. This results from a membrane forming on the outside of the implant and another on the inside of the body.

Heavily textured implants also have a stronger link to a very rare tumor form, BIA-ALCL, which has gained attention recently.

Implants

Implant size

To find the right implant size, we measure the width of the chest, the placement of the nipple, and the volume of the natural breasts. We also assess the skin quality, chest shape, and breast fold placement. All these factors affect the result, and it is important to be aware of your own conditions from the start.

The width of the implant should be adapted to the body’s conditions and your preferences. If you desire a natural-looking result, the implant size should match the width of your chest. If you prefer a more exaggerated and unnatural result, the implant size should be large enough to extend beyond the chest and also higher on the chest wall.

One factor that is challenging to influence is the distance between the breasts in the middle, known as the cleavage. The width of the cleavage depends on how close the breasts are naturally and how much breast tissue and subcutaneous fat you have to cover the implant.

How does a breast augmentation work?

A consultation appointment with one of our plastic surgeons is booked for 30 minutes. We will review your health and any specific considerations regarding anesthesia and surgery. The most important aspect is that you gain a good understanding of your conditions for the surgery and what you can expect.

It is beneficial to prepare by knowing the results you desire and bringing pictures to your consultation for discussion.

We will examine and measure your breasts to have a good starting point and to advise you on the best possible approach.

Usually, one visit before the surgery is sufficient, but if you feel uncertain about anything, you can come back as many times as you need to feel secure about the surgery. We also maintain contact before and after the surgery so that you can ask questions directly to your plastic surgeon or get help from our dedicated staff.

On the day of your surgery, you will arrive at the clinic, take a shower before the operation, and then dress in the clinic’s robe and rest in a bed. We do everything to minimize the risk of complications, including maintaining strict hygiene.

You will receive sedative and relaxing medication if desired, as well as pain relief to ensure you are as comfortable as possible during and after the surgery.

We prefer that you stay overnight after a breast augmentation to ensure the best possible result and safety. This is included in the cost of the surgery.

You will meet with your plastic surgeon or nurse for a follow-up 1-2 weeks after the procedure, depending on the specifics of the surgery. After this, we stay in contact to provide advice on how to take care of yourself in the best possible way.

You will schedule an appointment that suits you about 3 months after the surgery to evaluate the results and see an early final outcome. It takes about a year before the results can be considered more complete regarding scars and the body’s full adaptation to the changes.

Complications and poor results

Bleeding, infection, and wound healing issues are the most common surgical complications. While the risk is minimal, for those affected, it can be a significant challenge. We do everything possible to prevent and avoid complications, and it is equally important for the patient to follow pre- and post-operative care instructions.

Smoking doubles the risk of all types of complications. For the body to heal as effectively as possible, a smoke-free period of four weeks is required. Alcohol also increases the risk of healing problems, so it is important to be very restrained about two weeks before the operation.

Double bubble (DB)

DB, or Double Bubble, occurs when the breast does not appear as a single unit. Instead, the breast hangs over and has a visible remnant of the old breast fold. This risk is highest if the breast has poor skin quality, the fold is tight, and the implant lies below the breast fold. Sometimes it can be resolved by placing the implant in front of the muscle instead of behind it, but often a breast lift is required to correct the old fold.

Bottoming out

This means that the breast stretches at the bottom, causing it to sink, making the lower part of the breast thinner. This can be difficult to resolve as it involves the inherent strength of the body’s tissues, but it may be necessary to change the implant, stitch the pocket where the implant lies, or perform a breast lift.

Muscle impact

This can mean various things, but in terms of undesirable results, it involves both form and discomfort. All breasts and implants move with the activity of the chest muscle.

A typical effect of implants behind the chest muscle is that they are pulled upwards and outwards when the muscle is tensed, deforming the breast, which returns to its shape when relaxed. An implant in front of the muscle lies in contact with the front of the chest muscle and also moves when the muscle is tensed but does not deform in the same way. In rare cases, this is associated with pain when using the muscle. If the discomfort is significant, it may be necessary to move the implants and place them in front of the muscle.

When it comes to complications and undesired outcomes, they are covered by the clinic’s guarantee and will be corrected free of charge. It is important to understand that not everyone can achieve the same results, and the goal is the “best possible result.” During the consultation, we will point out what is specific to you and if there is anything in your particular circumstances that we need to pay special attention to.

Rippling

Rippling as an undesirable result means that noticeable waviness appears on the implant. When the implant lies on a smooth surface, it has a smooth form, but when placed in the body, it is always wavy at the edges. Implants can have different shapes, round or teardrop-shaped, and can be of varying firmness, but all implants have rippling in the body.

If the implant is covered by skin and muscle, this is not noticeable, but if the area between the implant and the skin is thin, you can more easily notice the irregularities. It is important to consider this in advance when choosing implants, and the most significant factor that increases the risk of noticeable rippling is the size relative to the body’s measurements and skin quality.

Kvalitetskontroll

Reformkliniken follows up and evaluates the results of operations and has a systematic quality assurance process to detect any potential problems.

To track post-operative development and identify any issues that may arise, Reformkliniken and other reputable clinics report to BRIMP, the national breast implant register.
